MEDICAL PLAN
The
Medical Plan is offered through
UnitedHealthcare. An employee is
eligible to participate in the
Medical Plan the first of the month
following one month of employment if
he/she is budgeted to work at least
15 hours per week.
There are three Medical Plan
Options, which vary in premium and
out-of-pocket expense: 1) Healthy
Choice Gold; 2) Healthy Choice
Silver; and 3) Healthy Choice
Bronze. Each plan option allows the
covered employee to decide whether
to use a Bon Secours facility, UHC
provider, or an out-of-network
provider.
If an employee chooses not to select
a Medical Plan Option, and has
coverage under another group plan,
he/she is eligible for an Opt-Out
Credit.
PRESCRIPTION DRUG INSURANCE
Prescription Drug coverage is
provided through Express Scripts for
employees who enroll in one of the
Medical Plan Options. Co-pays vary
depending on whether the medication
is Generic, Brand Formulary, or
Brand Non-Formulary.
DENTAL
INSURANCE
The Dental Plan is offered through
MetLife. An employee is eligible to
participate in the Dental Plan the
first of the month following one
month of employment if he/she is
budgeted to work at least 15 hours
per week.
The Dental Plan is a Preferred
Dentist Program (PDP) which means a
covered employee can decide to use
an in-network provider or an
out-of-network provider. Covered
services include:
- Preventive and Diagnostic
- Restorative
- Orthodontia (for children and
adults)
VISION
INSURANCE
The Vision Plan is offered through
Spectera. An employee is eligible to
participate in the Vision Plan the
first of the month following one
month of employment if he/she is
budgeted to work at least 15 hours
per week.
Employees may choose the Core Option
or the Buy-up Option, which offers
enhanced benefits at an increased
premium. Both plan options provide
coverage for:

FLEXIBLE
SPENDING ACCOUNTS (FSAs)
Flexible Spending Accounts are
offered through ConnectYourCare. An
employee is eligible to participate
in the FSAs the first of the month
following one month of employment if
he /she is budgeted to work at least
15 hours per week.
FSAs allow an employee to set aside
tax-free dollars from his/her
paycheck to pay for approved health
care expenses not covered by
insurance (such as co-pays and
deductibles) or dependent care
expenses (such as licensed day care
expenses).
Maximum annual contributions are as
follows:
Healthcare FSA – $5,000
Dependent Care FSA – $5,000
